SameSky Health launches Diabetes Care Pathway with Tufts Health Plan, a Point32Health Company
Pathway focuses on educating and guiding Tufts Health Direct members with a diabetes diagnosis.
North Hollywood, CA • February 13, 2023 — Today, SameSky Health Inc., a cultural experience company, announced the launch of its Diabetes Care Pathway with Tufts Health Plan, a Point32Health company.
The Diabetes Care Pathway focuses on educating and guiding Tufts Health Direct members with a diabetes diagnosis on their personalized journeys to manage their diabetes, including proper foot care, blood pressure control, and essential health checkups such as their A1c kidney screenings or retinal eye exams. Tufts Health Direct is a qualified health plan for a range of individuals and families some of which are state and federally subsidized. Qualified members will receive weekly messages via text to help them navigate their care.
